Chizuk Amuno Membership Directory
With the underlying hope of deepening connections with others in our synagogue community, two years agao Chizuk Amuno published its first Membership Directory. Due to its overwhelmingly positive response, we are planning to reissue the directory every other year. This directory includes contact information for all memebers of our synagogue community who choose to be listed.
Every member of the congregation will be receiving a letter in the mail regarding their current information. We ask that you review this information and ensure that it is correct. If changes need to be made you have the option of either filling out the form below, or mailing in the corrections. You may also choose to not be listed in the directory by again, filling out the form below, or mailing in your request. If our office does not receive any information from you, your information will be listed in the directory as it is shown on the letter you received via mail.

Directions for using the online form:
- Your membership number is required to complete this form, please find it on the bottom right corner of the letter you received regarding your directory listing. (Please note: If you do not have your membership number please call (410-824-2057) or email (a29zaGFAY2hpenVrYW11bm8ub3Jn) the development office.)
- Once you have entered your membership number you need only fill out the portions of the form which indicate a change. (i.e., if your phone number is listed correctly on your letter, you may leave that section blank)
- If you do not wish to be listed in the directory, please provide your membership number and then check the box indicating you do not wish to be listed. No other information is nessecary.
- If you would like to omit a section(s), please list in the space provided at the end of the form.
